Brown-Sequard syndrome has been rarely reported as a complication of interlaminar cervical epidural injection (ICEI). We report a 60-year-old male patient who presented with left upper and lower extremity weakness, right upper and lower extremity decreased pin prick and temperature sensation, left upper and lower extremity decreased touch and vibration sensation after ICEI. C-spine MRI revealed epidural hematoma in C5 to C7. But, intraoperation findings were subdural, intramedullary hematoma in C5 to C7 and left side direct spinal cord injury in C5. After decompressive operation, patient¡¯s symptoms improved but motor and sensory symptoms persisted. We report one case of Brown- Sequard syndrome due to direct spinal cord injury and subdural, intramedullary hematoma after ICEI.
